NTSB summary

HISTORY OF FLIGHT On November 30, 1993, about 1550 eastern standard time, a Mooney M20E, N9241V, registered to Frank B. Houghton and Jimmy R. Pfieffer, operated by the North Florida Flight Center, crashed about 6 miles south-southeast of the Craig Municipal Airport, Jacksonville, Florida, while on a 14 CFR Part 91 personal flight. Visual meteorological conditions prevailed at the time and no flight plan was filed. The airplane was substantially damaged and the private pilot, the sole occupant, was fatally injured. The flight originated at 1311, from the Craig Municipal Airport, Jacksonville, Florida. On November 28th, 2 days before the accident, the accident pilot had operated the accident airplane for 1.1 hours as indicated by the Hobbs meter. After landing the fuel tanks were filled and the airplane was parked. The airplane was not operated after the flight on the 28th or before it was rented on the day of the accident by the accident pilot. After the flight was cleared for takeoff there was no further contact with the Craig Municipal Airport Tower. According to several witnesses near the accident site, the airplane was seen before the accident flying no more than 1,000 feet above ground level in what was described by one witness as "erratic." The airplane was observed to pitch slight nose up then pitched nose down and impacted the ground in an undeveloped planned residential area. PERSONNEL INFORMATION First pilot information is contained in NTSB Form Factual Report-Aviation. AIRCRAFT INFORMATION Airplane information is contained in the NTSB Form Factual Report-Aviation

Probable cause

FAILURE OF THE PILOT TO MAINTAIN ADEQUATE AIRSPEED, WHILE MANEUVERING AT LOW ALTITUDE, WHICH RESULTED IN AN INADVERTENT STALL. A FACTOR RELATED TO THE ACCIDENT WAS: THE LACK OF ALTITUDE FOR RECOVERY FROM A STALL.

Phase and cause come from the NTSB’s own coding of each accident, which only exists from 2008 on, so these two filters leave out 1982–2007. Cause categories are only recorded for some accidents, and early findings can change.
