NTSB summary
HISTORY OF FLIGHTOn August 26, 2024, about 0931 central daylight time, a Piper PA-28-140 airplane, N9626K, was destroyed when it was involved in an accident near Wayne, Nebraska. The pilot was fatally injured. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. According to the manager of the fixed base operator (FBO) at Norfolk Regional Airport (OFK), Norfolk, Nebraska, the pilot arrived in the accident airplane sometime between 1100 and 1200 on Friday, August 23. The airplane was stored in a hangar over the weekend and the pilot returned Monday morning (the day of the accident). The FBO manager stated that the pilot obtained fuel from the self-serve pump before departure. A copy of the fuel receipt showed that the pilot purchased 25 gallons of 100LL aviation gasoline. No fuel quality issues were reported. According to the friend the pilot was visiting for the weekend, the pilot seemed rested and reviewed weather on one of his tablets before they left for the airport on the morning of the accident. Flight track information for the accident flight was derived from PASSUR OpsVue NextGen track data (OpsVue), a multi-sensor flight tracking system that integrates FAA radar, ADS-B, airport surface surveillance, and flight plan data. Altitude information may vary by several hundred feet and should be considered approximate; however, altitude trends and rates of climb and descent are considered reliable. The first data point showed the airplane about 4 nautical miles northeast of OFK at 0916:52, at an altitude about 2,450 ft above mean sea
Probable cause
The non-instrument-rated pilot’s encounter with instrument meteorological conditions, which resulted in spatial disorientation and a loss of control.
